This publication was published by the Centers for Disease Control in December, 1996 and generally contains data through 1992. A brief narrative section (which includes some charts and graphs) precedes the main portion of the atlas, which is a series of nearly 100 maps. These maps show age-adjusted death rates by Health Services Area from more than 20 causes, with separate maps for white males and females and black males and females. There are maps illustrating the following causes of death:

The narrative section of the Atlas discusses methodology and specific details and highlights for each cause of death.
